Weighing food; cooked or raw?

All the (many) guides I've been finding online for how to weigh and track your food never really address my big question...should I weigh my food cooked or raw? I usually do it raw, but then, when I've added a bunch of ingredients and cooked them, I'm not sure how to measure or weigh the portion of cooked food? I'm trying to be as accurate as possible!
