Having trouble going past 1000 calories

I literally can't seem to cram enough calories into myself per day, my metabolism has slowed down from what it was in my teenage years. I can reach 60 grams of protein and usually 30g of fat but I rarely go above 80g of carbs and my calorie intake varies from 800 to 1200 a day. Am I eating too lean? I've gained 5 lbs in 6 years but my muscle definition and size has gone from string bean to a decent physique I think. Am I just some kind of medical anomaly or am I just barely getting enough calories to maintain and grow small amounts of muscle? I also have a family history of heart disease and high blood pressure so that is why I try to stick to lean meat and healthy fats, whole sources of carbs. I've noticed huge gains when I increase weight and stick to the 4-6 sets of 10-12 reps upper body and 4-6 sets of 12-14 reps lower body.

I went from pressing 20 lb dumbbells to 35 lb and it seemed like my muscles exploded even after lifting the same weight for so long, this has to be the key to getting bigger right?

In a nutshell if I'm seeing lean muscle gains and maintaining the same body fat % do I just need to increase weight incrementally? Dirty bulking is not an option due to my heart risks.
