What is insulin sensitivity, really?

I understand the basic concept: it's the ability, or lack thereof, your cells to uptake insulin at the receptor site...

But, why would it be higher at certain times, and lower at other times in the same individual?

I know that intramyocellular lipids (fats within the cell) can decease it, but in an acute sense... like AM vs PM... why would it be higher during one period vs another?
