Returning to the gym. What usually happens?

Been off the gym for like 5 years, lost alot of muscle and added fat. Went from 210 to 280 (6 feet 3 inches tall) after some meds that are known to mess upp metabolism and I started to eat like a maniac.

But what is usually happenning when u return to the gym? I seem to be adding muscle and not loosing fat on a huge deficit. Am I just getting my old sleeping muscles back before my body starts to burn or how does it usually go?

And yes, I can count calories and have done so successfully in the past so this thread is more a question about what happens to the body as it adapt to lifting again if you previously had muscle.
