Body fat set point theory questions

I’ve been looking into this lately and wonder, what do you guys think about the theory that each individual has their own “comfortable” body fat level where appetite and energy levels are optimal?

At what age is this set point determined? Adolescence or childhood?

Why is it that many Americans become morbidly obese despite this theory? Surely there shouldn’t be any human body that feels comfortable at 45% body fat. Are these people constantly force feeding against their own appetite or is it a psychological issue?

Does this set point change throughout life or remain the same?
